Account grouping by institution is incorrect

On the Accounts page, using the "Group by: Institution" filter option returns the same results as "Group by: Connection". If accounts from the same institution were added individually, they will not group, despite having the same Institution Name.
Also, when using the "Group by: Connection" filter, manually-managed accounts are all listed together under a single "connection", and the header for that "connection" is taken from the first account in the list. Instead, it should just say "Manually-managed accounts" instead.
